Basic semen analysis

Abstract

For males seeking fertility assessment, a baseline semen analysis is the initial test. Overall, from both a clinical and research standpoint, the World Health Organization (WHO) manual’s 5th edition has significantly advanced the field of male infertility, and the inclusion of data from reference subsets is a part of a continuing effort to quantify male fertility. The newly released 6th edition, which was released in July 2021, emphasizes the value of semen analysis as a tool to: (1) aid in the diagnosis of fertility/infertility; (2) assess male reproductive health and function to guide management; (3) guide the choice of assisted reproductive procedure; (4) monitor response to treatment; and (5) measure the effectiveness of male contraception. The removal of baseline values is a significant deviation from the 5th version. This 6th edition makes it very obvious that using the 5th percentile readings alone is insufficient for diagnosing male infertility. In this chapter, we describe the technical principles of basic semen analysis, outline the main etiologies associated with identified sperm abnormalities, and summarize what the physician can expect from basic semen analysis.
